Thank you for starting this thread. I looked at the DS11 form and I’m unsure how to navigate the 3 eligibility requirements: #1) applying for first passport #2) under 16 yrs #3) previous passport issued before age 16 or issued more than 15 years ago or passport lost, stolen, or damaged. Do folks move forward with this regardless? And in terms of next steps, is it as straightforward as making an appt at a passport agency, bringing DS11 and required documents (do you also bring the affidavit?), and submitting the information? I want to move forward with this process but need more information, if possible (and if this has already been discussed, please feel free to direct me to prior resources).
