    Dear Dr Lando,

    I started following you and Mike back in 2019/20 and what a ride it has been. I previously practiced for many years as a dentist and oral surgeon, and I was constantly in trouble for pointing out the obvious. My antics would have brought a smile, I’m sure.

    The reason I am writing is that I developed an umbilical hernia back in 2012, which recently opened up whilst doing Pilates reformer classes of all things, and I have chosen to have it repaired. I have no desire to have a general anaesthetic, but the surgeon can’t perform the procedure under LA.

    As you will be aware, there are various component in anaesthesia that are best avoided, and potential after effects also. I’m wondering if there are any protocols you would recommend – other than not having one! I’m scheduled for surgery on the 2 April.

    I would be grateful of any pointers.
